To be eligible for nomination, an athlete must have graduated from the Midview Local School District, a minimum of ten years prior to being considered for enshrinement. The eligibility for team nominations will be a minimum of five years. School personnel (coaches, athletic directors, trainers, etc.) are eligible at the discretion of the committee. There shall be no time limit or restriction of those individuals nominated strictly for their service to Midview Athletics.

Special Recognition

Nominee must have made immeasurable contributions of time and effort to the enhancement of Midview Athletics for the benefit of Midview students. Provide information on reverse side to support nomination. Provide any further documentation e.g. newspaper clippings or other recognition which will be of benefit. Nominee is not required to have been a Midview student.
