Step 1:	Review the ► By-Laws ◄ for Athletic Nominees which may include an individual, coach, or team. Special Recognition Nominees must have made immeasurable contributions of time and effort to the enhancement of Midview Athletics for the benefit of Midview students.
Step 2:	Go to the ►Nomination Application◄ page, download and print out the forms. or Go to the ►Online Nomination Application◄ page, complete and submit the Online Nonimation Application.
Step 3:	Fill out form in its entirety.
